What's Included in the SNF Dialysis Program

A complete in-facility dialysis program — equipment, staffing, oversight, and compliance — under one KidneyCare contract

In-Center Hemodialysis On-Site

Portable, ICU-compatible dialysis equipment delivered and operated by our licensed dialysis nurses. 3x weekly schedule (or as ordered) integrated with your facility's daily flow.

Isolation-Capable Workflow

Dedicated workflow and equipment for HCV, MDRO, and COVID-positive residents — so isolation status doesn't become a barrier to accepting an admission.

PD & Home-Dialysis Management

Residents already on PD or home hemodialysis stay on their modality. Our team manages it in your building. No need to build separate PD or HHD competency in-house.

Dedicated Clinical Team

Same dialysis nurses visit your facility on a fixed schedule. Board-certified nephrologist supervises every treatment plan with monthly resident evaluations.

Documentation & Compliance

CMS-compliant treatment records integrated with your charting system. Survey-ready documentation for state inspections and Five-Star quality reporting.

Emergency Coverage

24/7 on-call nephrologist for urgent dialysis needs, missed treatments, or access complications. Reduces ER transfers and rehospitalizations.

What Adding On-Site Dialysis Does for Your SNF

  • Admit dialysis-dependent residents you previously had to decline
  • Eliminate ambulance transport costs (~$300–800 per round trip, 3x weekly)
  • Reduce rehospitalization rate — a CMS Five-Star quality measure
  • Accept HCV/MDRO/COVID-positive dialysis patients with isolation workflow
  • Improve length-of-stay metrics and discharge planning
  • No capital investment in dialysis equipment or build-out
  • Differentiation in your market vs. SNFs without on-site dialysis

Frequently Asked Questions

What's the difference between this and your /for-skilled-nursing-facilities partnership page?

This page describes the dialysis service itself — modalities, staffing, equipment, isolation workflow. The partnerships page describes the business model — onboarding, scheduling, what your facility provides vs. what we provide. They're complementary.

What space and utilities does our SNF need?

A dedicated treatment area (typically 200–400 sq ft for 2–4 stations), a water source meeting AAMI dialysis-quality standards (we provide the RO water treatment system), and adequate electrical capacity. We do a site assessment as part of onboarding.

How does isolation dialysis work in our building?

We rotate isolation patients into dedicated time slots with separate equipment and a strict workflow. For SNFs with HCV-positive residents, we maintain dedicated machines per CDC standards. This means you can accept admissions that other SNFs decline.

Will this affect our CMS Five-Star rating?

Indirectly, yes — on-site dialysis tends to reduce rehospitalization rate and ER transfers (both Five-Star quality measures) by eliminating the transport-and-wait failure modes. We help you track and report the relevant metrics.

What about residents who only need dialysis temporarily (AKI recovery)?

Same program. Our team adjusts modality and schedule as the resident's kidney function recovers. We coordinate with their nephrologist for the eventual discharge from dialysis if appropriate.

How long does it take to launch the program?

Typically 2–6 weeks from contract signature to first treatment, depending on space readiness and water-treatment install. We can also do interim mobile-based service for urgent admissions during build-out.
