How quickly can you place a dialysis patient in an SNF?
For patients without complex isolation needs, we can often identify bed availability same-day. Actual placement depends on the receiving facility's admission process, but we expedite by having dialysis already arranged. Complex cases (HCV, MDRO) may take 24-48 hours as we match to appropriate facilities.
What information do you need to start the placement process?
Patient demographics, dialysis prescription (if established), current dialysis access type, any isolation requirements, and insurance information. We can start with minimal info and gather details as we work. The sooner you call, the sooner we can help.
Do you handle patients who aren't on dialysis yet but will need it soon?
Yes. If a patient is progressing toward dialysis need (rising creatinine, anticipated dialysis start), we can still place them at our partner SNFs. We'll manage dialysis initiation when the time comes, keeping the patient in place rather than requiring transfer.
What about patients going home who need outpatient dialysis?
We can help schedule outpatient dialysis and coordinate initial appointments. For patients going home on PD or home HD, we provide training and equipment support through our home dialysis program.
How do you handle insurance verification?
We verify dialysis coverage and coordinate with your utilization review team. Most commercial, Medicare, and Medi-Cal plans are accepted at our partner facilities. We flag any coverage issues early to avoid discharge delays.
